Modified setup package to allow configuration of up to 5 custom folders instead of the previous single shared location.

Upgrading will import existing shared folder location as Custom Folder 1.

* Note: The core applications in this version are exactly the same in this version as 1044 - the changes are specific to the setup package and modifications to the configuration files only.
